How do I convert units to mg?

Units on an insulin syringe measure volume, not weight. On a U-100 syringe, 1 unit = 0.01 mL. To get mg, multiply the volume by the concentration: mg = units ÷ 100 × (vial mg ÷ water mL). Example: 20 units from a 10 mg vial mixed with 2 mL = 0.2 mL × 5 mg/mL = 1 mg.
How many mL is 10 units?

On a U-100 insulin syringe, 10 units = 0.1 mL, 25 units = 0.25 mL, 50 units = 0.5 mL and 100 units = 1 mL. This is true for any liquid; only the amount of drug in that volume depends on concentration.
Why is there no fixed units-to-mg conversion?

Because the same volume can hold very different amounts depending on how much water was added to the vial. 10 units of a 5 mg/mL solution is 0.5 mg, while 10 units of a 2.5 mg/mL solution is 0.25 mg.
